Medication adherence: using 365-day data to catch gaps
Medication adherence is easy to measure after the fact and hard to fix once a patient has already lapsed. A rolling 365-day view of fill history, scored by AI, moves the signal earlier, so the care team can intervene before a gap becomes a missed refill.
Why a single data point is not enough
A most-recent fill date tells you what happened last, not where the patient is heading. A full year of fills, refills, and gaps plotted against the expected schedule reveals a slow drift toward non-adherence long before a prescription formally lapses.
What AI adds
The model reads the pattern building up to a missed refill and produces a rolling risk estimate for each patient. Instead of reacting to an abandoned refill, the care team sees at-risk patients surface in the workflow while there is still time to act.
Turning prediction into action
Predictions are only useful if they reach the right person at the right time. In H2H DigitalRx, at-risk patients appear directly in the prescriber workflow, and outreach or a refill authorization can happen from the same screen, with no separate report to pull.
