What is Medical Adherence?
Medical adherence, or the extent to which patients take their medications as prescribed, is a critical aspect of effective healthcare.
The World Health Organization defines Medical Adherence as “the degree to which the person’s behavior corresponds with the agreed recommendations from a health care provider”
Problem Statement
Medication adherence is a major challenge in healthcare, with studies showing that non-adherence leads to poorer health outcomes, increased hospitalizations, and higher healthcare costs.
Providers need better insights to understand patient adherence to prescribed medications, to ensure optimal treatment outcomes.
Enhancing Care with the Medical Adherence Module
- The Medical Adherence Module enhances the H2H Digital Rx platform by providing real-time insights into patient medication adherence, along with factors influencing it, based on prescription refill history.
- The Medical Adherence Module automatically analyzes refill data to deliver an adherence percentage for each patient to the provider.
- The Medical Adherence Module provides providers with suggestions to address factors impacting patient adherence and enables them to offer additional support or counseling, emphasizing the importance of adherence to keep patients on track with their treatment.
- The Medical Adherence Module sends medication reminders to patients to help them adhere to their prescribed treatments
How this Works?
- The Medical Adherence Module is based on the patient’s past adherence, medication history, and refill request timing after completing their current medication regimen.
- It analyzes data such as prescription dates, time taken to complete the regimen, and refill requests made by the patient.
- The AI evaluates adherence behavior, identifies factors contributing to non-adherence, and calculates an overall adherence percentage for each patient and each prescribed drug.
- The module provides insights into adherence-impacting factors and offers suggestions for providers to address these issues.
- Providers can use these insights to offer additional support or counseling, emphasizing the importance of adherence.
- Additionally, the module sends medication reminders to patients to help them stay on track with their prescribed treatments.

Steps to use Medical Adherence Module
-
- Step-1: Log in to the e-prescription platform
- Step-2: Prescribe the medications on the New Rx pad as usual, and proceed to the Prescription Summary screen.
- Step-3: On each drug prescription pane, there is an AI icon displayed at the lower right corner, showing the percentage of the patient’s medical adherence.
- Step-4: Click on the ‘Find more details’ text in the pane to view the AI Prediction pop-up, which provides a detailed overview of the medication adherence percentage, adherence rate, factors impacting adherence, and suggested actions. The medication adherence score is categorized into three levels: Low, Moderate, and High.
The screen below shows that the probability of the patient adhering to their medication is high, based on their medication and refill history.
The screen below shows that the probability of the patient adhering to their medication is low (below 50%) based on their medication and refill history. The AI prediction screen provides reasons for this and offers suggestions to help the provider advise the patient on the importance of medication adherence. Our AI algorithm delivers insights to the provider based on the patient’s medication history and their promptness in refilling prescriptions, allowing the provider to recommend actions to help the patient stay on track.
The screen below shows that the probability of the patient adhering to their medication is moderate (between 50% and 80%), based on their medication and refill history. The AI prediction screen provides reasons for this and offers suggestions to help the provider advise the patient on medication adherence. It also helps the provider make necessary adjustments to the prescription if the patient experiences any discomfort with the prescribed medications.
Below the suggestions, there are ‘Agree’ and ‘Disagree’ buttons that providers can use to provide feedback which in turn improves the future predictions.
Note: The medication adherence score displayed is generated by an AI model. Therefore, it is the prescriber’s responsibility to make informed decisions when modifying medications or providing other recommendations.
