Medication reconciliation is a structured clinical process that ensures a patient’s medication list is accurate, complete, and up to date across all points of care. Its primary goal is to prevent medication errors such as omissions, duplications, dosing mistakes, or harmful drug interactions—particularly during transitions of care, like hospital admissions, discharges, or changes in healthcare providers.

In H2H Digital Rx, medication reconciliation is supported through the Medication History view. This allows healthcare providers to systematically review prescribed medications and reconcile them against the patient’s current medication list, ensuring continuity of care and patient safety.

Within H2H DigitalRx, medication reconciliation is seamlessly integrated into the Medication History module to support accurate and up-to-date patient medication management. This functionality enables prescribers to review a consolidated list of the patient’s current and previously prescribed medications, ensuring a clear understanding of ongoing therapies before making clinical decisions.

By offering a comprehensive and organized medication history, H2H Digital Rx helps healthcare providers ensure patient safety, prevent medication errors, and make informed clinical decisions. The module streamlines the reconciliation process, allowing prescribers to efficiently review, update, and manage patient medications across all points of care.

From the Medication History view, prescribers can select one or more medications that require reconciliation. This may include medications that are no longer clinically appropriate, have been replaced by alternative therapies, or were prescribed in error. The system provides the ability to reconcile these selected medications and initiate a CancelRx request directly from the same interface, eliminating the need for manual communication with the pharmacy.

When a CancelRx request is submitted, H2H DigitalRx electronically transmits the cancellation to the designated pharmacy in accordance with NCPDP SCRIPT standards. This helps ensure that discontinued medications are promptly removed from the pharmacy’s active dispensing queue, reducing the risk of unintended dispensing and improving patient safety.

This reconciliation workflow enhances medication safety by minimizing discrepancies between active prescriptions and the patient’s current treatment plan. It supports regulatory and quality requirements by maintaining a complete, traceable medication record. How Medication Reconciliation Works

1. Access Medication History

  • Navigate to the patient’s profile and open the Medication History tab.
  • View both current and previously prescribed medications, including dosage, frequency, and prescribing provider.


  • 2. Select Medications for Reconciliation

  • From the Medication History view, prescribers can select one or more medications that require reconciliation.
  • This may include medications that are no longer clinically appropriate, have been replaced by alternative therapies, or were prescribed in error.


  • 3. Reconcile Medications and Initiate CancelRx

  • The system provides the ability to reconcile selected medications directly within the interface.
  • Prescribers can initiate a CancelRx request for discontinued or incorrect medications without the need for manual communication with the pharmacy.


  • 4. CancelRx Transmission

  • When a CancelRx request is submitted, H2H Digital Rx electronically transmits the cancellation to the designated pharmacy in accordance with NCPDP SCRIPT standards.
