H2H DRx Patient Portal
We are pleased to introduce the H2H Digital Rx Patient Portal, a new enhancement to the H2H Digital Rx platform. This feature is designed to improve patient engagement, streamline communication, and provide a seamless prescription access experience.
The Patient Portal enables patients to securely and conveniently access their prescription information through an intuitive, user-friendly interface. By enhancing visibility and reducing communication gaps, it supports better care coordination and an overall improved patient experience.
To enable patient portal access, providers must opt patients in to receive email notifications on the patient record within the H2H Digital Rx platform. This option is available only for patients with a registered email address. If a patient does not have an email address associated with their record, the provider can add one on the patient’s behalf.
Once a patient is opted in, they will receive an email notification whenever a prescription is sent to the pharmacy. This email includes a link to the Patient Portal. By selecting “View My Prescription,” patients are directed to the portal login page.
From the login page, patients can access their prescriptions by entering their last name and date of birth (DOB) associated with their Digital Rx account.
In the Patient Login, prescriptions can be viewed pharmacy-wide along with the total count. It also displays additional information such as Get Direction and Call Pharmacy.

By clicking on Get Directions, it will navigate to the map to view directions to the pharmacy, helping the patient reach the pharmacy easily to obtain their medications.
From the Pharmacy dropdown, patients can select the required pharmacy and view the prescriptions prescribed to that pharmacy.
